Through triage and screening, GroundZero conducts an intensive process of moving people forward within 30 days into programs and places that contribute to their success.
TRIAGE: The First 30 Days
During the first 30 days, attendees are in an evaluation phase to determine if the program is a good fit for them.
Double Evaluation: The double evaluation process assesses both the attendee's willingness to continue and their readiness to benefit from the program.
This phase allows individuals to self-evaluate their commitment and progress while our team evaluates their suitability for further participation.
Participants who continue work closely with their triage case worker to implement their personalized case plans and address specific areas of need.
Before discharge, attendees meet with the Care Team one more time to review their progress, achievements, and future plans.
